what is the word for word introduction for a sample recording?

Good evening, can someone post the exact wording of an introduction to a piece being read? As in title, author, reader's name and livrivox disclaimer?

It partly depends on what type of project it is - poetry, a solo book, or a group book. If you look at the first post of any project thread, it'll show the wording.
