Ok, here's my stab at it:
Last name:
De Voocht: De Foe-gt (try to roll that g in your throat for a guttural sound; not easy to do, I know)
Last name:
Van Aitzema: Fun Eight-zuh-ma
Word for letters:
Brieven: Breefen
Place name:
Amersfoort: Ah-mers (rhymes with pears) -fort
First name:
Nicolaes: Nee-coal-aye-ess
First name:
Kaatje: Kaah-t-yuh
River name:
Ijssel: eh-ee (practice these vowels separately, then say them faster to blend them together so you still hear both) -ssell
Moor or field name:
Veluwe: Fay-loo-wuh
Last name:
De Keysere: De Kay-sir
Place name:
Ede: A-duh (the A rhymes with fay)
