NEW PROJECT TEMPLATE GENERATOR Issue

I'm trying to submit a new project, but once I fill out the NEW PROJECT TEMPLATE GENERATOR and hit Submit, all I get is a blank screen. There are no red areas. What am I doing wrong?

Two possibilities to explain this -

The author is not in the database

In some field you have entered non-alphnumeric characters where they are not allowed.

I couldn't launch a new project because the author wasn't in the database.

Could an admin add Samuel Miller for me? Thankyou!
