You just read the creed without the commentary and I would read the alternate translations in brakets and use intonation to make clear that it is a parallel phrase rather than an addition.
For the intro, read "Creed of Gregory Thaumaturgus, of Neo-Caesarea A.D. 270 by Gregory Thaumaturgus" etc.
