Quick question: Should the Transcriber's Note at the end be included in the recording? It looks to be about the front cover image. If I should record it, does it need to be its own section, or included at the end of Chapter 12?
I'll give you a quick answer which in "no." These are NOT part of the actual book, but notes made by the person who created the Gutenberg ebook.

Well, I think key words are a challenge too.

I thy to think of words or terms someone might use to search for a book that might make this one pop up. That can be hard for a fiction book. But, here are just a few that come to mind:

juvenile fiction, American West, Indians, Sioux, Montana

Ann can add some if you like. You might come up with a few more, but nothing to stress over.
People will find the book by genre as well.
