We can't use the double apostrophe in the MW ("). Instead we use two single apostrophes (''). As you can see they look very similar. The double apostrophe has a special meaning at Archive.org and causes problem when we send them our files. I changed the ones in the chapter two filename.

They look exactly the same! I had to look over and over again to figure out what you were saying. They shouldn't be in the notes or the chapter titles?
