I don't remember running into this sort of thing with the readings I have done. That is, so far!

We'll have to wait for guidance from the BC, but I would take a little bit of a liberty and say, for instance: Paradise Lost, Book 6, Page 174 for "Par. Lost. lib. vi. fo. 174." To make it very proper you could say "Liber 6, Folio 174."

For that matter, I think you would be safe in simply saying "Book 6" as we would not be using Defoe's text anyway were we to look for the passage. (Well, it's darn unlikely.)

"Dear B_____s" would be "Dear B blank s."

I'm sorry to disagree, but I don't believe these are initials. The text is with a capital 'B' and a small 's.' I only bring this up because to read 'B.S.' will sound silly as it stands for something else in today's time. Also, Defoe would be careful to hide his joking behind these blanks. To use initials would perhaps too clearly mark whom he was 'not' identifying. Again, my apologies.
