PL Note: the source has lengthy footnotes embedded in the middle of prepositional phrases so that the the objects of the prepositions lose all connection with the preceding parts of the phrase and sentence. So, In a couple of places I have moved the footnote text to the end of the phrases they're embedded in.

Also, I have recorded "End of footnote" to the end of each of the footnotes so as to clue in the listeners where the footnotes end.
