Dave,

I need your input on exactly where to end the book. After Chapter 13, there is a section titled "Notes" that should probably be included with 13. After that, there are some testimonials, footnotes, etc.

It's always the reader's decision but I would include only the "Notes" section. It could be added at the end of section 13.
