For example, chapter 2 would be "The Citizen's Almanac by U.S. Department of Homeland Security (2007) [ Rights and Responsibilities of US Citizens] and ad the end say "end of chapter 2" or insert the title again? Your choice entirely. Right now they would say "chapter 2 in both places".

Good, but the way you have it it looks like everybody will read "Chapter 2" etc. That is your example but the generic instruction comes first. I will show you how it is usually done., OK/ ? :D
